Best Whitening Toothpastes for Sensitive Teeth: What Does the Evidence Say?

Best Whitening Toothpastes for Sensitive Teeth: What Does the Evidence Say?

If you have sensitive teeth but would also like a brighter smile, choosing the right whitening toothpaste is very important. Some formulations can help remove surface stains while also providing necessary ingredients that manage dentinal hypersensitivity. However, not all whitening toothpastes work in the same way, and their effectiveness mainly depends on their ingredients.

How do whitening toothpastes work?

Most whitening toothpastes work primarily by removing extrinsic stains mainly caused by tea, coffee, red wine and tobacco.
These ingredients help gently polish the surface of the teeth, and some contain bleaching ingredients like hydrogen peroxide that help remove discoloration. However, peroxide-based bleaching is different from the surface stain removal achieved by the conventional whitening toothpaste.
It is therefore important to have realistic expectations: While whitening toothpastes can improve the appearance of stained teeth, they do not necessarily change the underlying natural colour of teeth.

Which ingredients should people with sensitive teeth look for?

Certain toothpaste ingredients have clinical studies to support their use to manage sensitivity in teeth.

  • Potassium Nitrate helps reduce the sensitivity response of the nerves.
  • Stannous Fluoride protects against tooth decay and can also reduce sensitivity.
  • Hydroxyapatite can also help reduce sensitivity by filling in dentinal tubules, and it has clinical studies that show its ability to help restore the minerals in enamel.
  • Calcium sodium phosphosilicate can help fill dentinal tubules and has shown to reduce sensitivity.

In a review and meta-analysis of 53 randomized controlled trials of over 4,700 participants, it was shown that some desensitizing toothpaste formulations were better than conventional toothpastes for relief.

In an additional network meta-analysis of over 120 trials of over 12,500 participants, the benefits of ingredients such as stannous fluoride, potassium formulations, and calcium sodium phosphosilicate were also evident.

The active ingredients of these toothpaste formulations should be taken into account rather than just trusting marketing statements.

How about the combination of whitening and sensitivity?

Whitening systems can have side effects. Bleaching systems that utilize peroxide can result in transient tooth sensitivity and possibly even irritation of the gums.
The American Dental Association identifies tooth sensitivity as a common adverse effect of bleaching, particularly with higher concentrations of peroxide.
People with any existing sensitivity should therefore consider a toothpaste formulated with recognised desensitising ingredients and must seek dental advice before starting any whitening treatment.
It is also very important to establish the cause of sensitivity. Discomfort can result from causes like enamel erosion, tooth decay, gum recession and broken teeth, all of which can require specialized care.

How do you choose the right toothpaste?

When comparing whitening toothpastes, consider:

  • Whether the formulation contains fluoride for cavity prevention.
  • Whether it includes an established desensitising ingredient.
  • Whether its whitening action is based on surface stain removal or chemical bleaching.
  • Whether the manufacturer provides credible clinical evidence supporting its claims.

Natural ingredients do not automatically mean a product is safer to use or more effective. Similarly, claims of enamel repair should be supported by clinical evidence rather than the ingredient descriptions alone.
